Retired murderer, John Wick(Keanu Reeves) closes in on his stolen 1969 Boss 429 Mustang at a place where stolen vehicles are disassembled and their parts sold which is owned by Abraham Tarasov(Peter Stormare), brother of Viggo and uncle of losef. John overpowers Tarasov's men in a violent fight that severely destroys his car. He then spares Tarasov under the name of "peace" and returns home.

While Aurelio (John Leguizamo) takes John's Mustang for repairs, Italian crime lord Santino D'Antonio(Ricardo Scamario) approaches John. it is revealed that, many years ago, prior to marrying Helen and retiring, John seeks D'Antonio's help to complete a herculean task. As an agreement between the both parties, John swore an unbreakable promise which emblems the use of a blood oath(Marker) medallion. The medallion is tendered by D'Antonio to demand services from John, but John declines, claiming that he is retired. John's house gets destroyed with a grenade launcher by D'Antonio, in retaliation.

John is reminded by Winston (Ian Mcshane), the owner of the continental hotel in New York City, that rejecting the oath(Marker), means violating one of the two sacred rules of the crime-world: no killing on Continental ground and Markers(oaths) must be honoured. John vetoes on his retirement and reconnects with D'Antonio, who assigns him to murder his sister Gianna D'Antonio(Claudia Gerini) so he can claim her seat among the council of high-level crime lords. D'Antonio sends Ares(Ruby Rose), his personal bodyguard to follow and keep an eye on John.

John penetrates Gianna's celebratory party in Rome and confronts her. Staring death in the face, Gianna chooses to commit suicide. To tie up loose ends, John is ambushed by D'Antonio's men, who attempt to kill him, as he retreats. After murdering most of them, Gianna's bodyguard Cassian(Common) chases John which culminates in a brutal fight which is halted when they crash into the reception area of the Rome Continental. Reluctantly, they share a drink and Cassian swears vengeance for Gianna's death.

Meanwhile, in New York City, D'Antonio opens a $7 million bounty(contract) for John's death which is followed by a spate of multiple attacks on him by other members of the underworld. Cassian confronts John once again at the subway. John wins the ensuing fight, but spares Cassian's life out of professional respect, though his death is all but assured by a knife to the aorta, which John tells him will kill him upon removal. Injured and desperate, John seeks help from underground crime lord, The Bowery King(Laurence Fishburne). His injuries are treated an he is guided to D'Antonio's location. John kills D'Antonio's men including Aries, but D'Antonio is able to escape to the Continental. D'Antonio makes it clear that he intends to seek indefinite refuge there and John, despite Winston's warning shoots and kills D'Antonio.

The next day, Winston tells John that, per the High Table, the contract on John has been doubled and offered globally. Thanks to Winston's influence, instead of being executed for killing on Continental ground, John is excommunicado and loses all access and privileges to underworld resources. Winston delays announcing John's excommunication by one hour to give him a head start, and provides him with a Marker for future use. Before leaving, John tells Winston to send a message that he will murder anyone who comes after him. John suspicious of everyone flees on foot with his dog.
